This Service User Survey can be used for current users of domiciliary care; their families, and friends;  to capture:

• views and experiences about the current commissioned domiciliary care provision at the care service (what works well and what could be improved);
• views about potential future service provision (related to what currently works well and what could be improved).

The survey can be completed over the phone or provided in paper form to obtain feedback on an annual basis.
